First-Time Applications
Required documents:
- Completed application Form 19 and two payment invoices.
- Original birth certificate and copy for citizens by birth.
- Original national identity card and copy for adults.
- Parents’ identity and birth records, as applicable.
- Parental consent form for minors.
- Additional documents as required by Immigration.
Passport Renewal / Replacement
Required documents:
- Completed application Form 19 and two payment invoices.
- Original national identity card and copy for adults.
- Previous passport and copy.
- Parental consent form for minors, where applicable.
- Additional documents as required by Immigration.
Lost Passport
Required documents:
- Completed Form 19 and two payment invoices
- Original national ID and copy for adults
- Written explanation of how the passport was lost
- Supporting sworn affidavit
- Police abstract reporting the loss
- Copy of the lost passport’s biodata page, if available
- Payment of the prescribed government fee
Damaged Passport
Required documents:
- Completed Form 19 and two payment invoices
- Original national ID and copy for adults
- Damaged passport and copy
- Written explanation of how the passport was damaged
- Supporting sworn affidavit
- Payment of the prescribed government fee

This is an independent administrative assistance charge payable to Luvisia Digital Limited. Government passport fees are charged separately according to the applicable passport category. Luvisia Digital Limited is not a government agency and does not guarantee passport approval or processing time.
